Opioid Health Home

Provide integrated, person-centered, and comprehensive care to eligible beneficiaries to successfully address complexity of comorbid physical and behavioral health conditions. The OHH team addresses all social determinants of health and provides comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support, and referrals to community and social support services. Qualifications are Medicaid recipients.
